W-9 Tax Form requests from Payors

18 STEPS

1. This tutorial demonstrates an example of a Payor who requests a W-9 tax form to be completed.

Responding to these requests is important, even if you have entered your Tax ID in Ref Insight Pay.

2. Some Payors only need the Tax ID information you entered on the Ref Insight Pay tab because they have a W-9 tax form already on file for you.

Important: Filling in the Tax ID information here is NOT a substitute for a signed W-9 tax form.

3. Some Payors need a signed W-9 tax form to meet their tax obligations, as a condition of payment, or both. If it is a condition of payment, you can only be paid if a signed W-9 is on file.

4. Our payment processing partner allows Payors to Request a W-9 form from officials via email.

The primary email in an official's profile is where this request will be sent. It will come from the address "no-reply@routable-mail.com".

5. The name of the Payor making the request for your tax information will also appear in the "From" line as well as in the Subject line.

6. Click Provide tax information to open the page where you will fill out the W-9 form and sign it. If you have a filled and signed IRS W-9 form on file, you can also upload it.

7. This is the page where you will be brought to. Click Add your W-9 tax information.

8. Most officials are individuals, such as contractors or freelancers. If you have set up a business for your officiating income, such as a sole proprietorship of an LLC, click "I represent a business" instead and fill in the form.

9. Type in your Legal First and Last Name.

10. Type in your Social Security Number.

11. Type in your Mailing address. The system will provide matches that you can click on to auto-fill the other fields. Please review any auto-filled fields to ensure the information is accurate.

12. If you already have a filled and signed W-9 tax form saved as a PDF on your computer and prefer to upload that file, choose Upload signed W-9 PDF.

13. If you choose this option, note that only an original IRS form in PDF form is accepted. Remember to type in the Date that the uploaded form was signed.

14. A more convenient option is to Sign electronically.

15. Type your First and Last Name in the signature box.

16. The Save and continue button will turn green after you type your name in. Click Save and continue to complete the process.

17. Your submission confirmation will display and the W-9 form will now be available to this Payor and only this Payor. Additional Payors may also request this form. It is important to respond to each request separately.
